Please fill out this form to become a Miami Valley Golf Volunteer. Once you submit this form, your profile will be created and I will be able to schedule you for shifts. 

Please Opt-in to receive text msgs so that you will receive shift reminders. 

When uploading a photo, please use a photo of yourself from the shoulders up. This photo may be used to create your volunteer badge. 

Once you submit the form and are approved, you will be able to access your profile from the My Profile button on the volunteer page. From there you can update your information and view shifts that you are signed up for.
